


Prior to your first appointment, please pre-register with us by clicking on the form names below and printing out each of our patient forms. Please fill them out at home and mail or fax (410.798.5165) them to our office at least 24 hours prior to your scheduled appointment. This allows our doctors time to carefully review your information prior to your visit.

We reserve time in our schedule for certain types of appointments. So, there’s no waiting weeks or months to be seen. Our schedule is carefully designed to allow us to spend quality time with each patient. Our new patient exam lasts 1 1/2 to 2 hours and our hygiene appointments are scheduled for a full hour.
We begin seeing patients at 7:30 am and our last appointments of the day begin at 2:30 pm, Monday through Thursday. We are closed for lunch between 12:30 and 1:30 pm.
Emergency care is available at all times. After hours, please call our main phone number (410.956.5555) to obtain the number at which you can reach our doctor on call.
If you must cancel or reschedule an appointment, we require at least 24 hours notice.

We do not participate with any insurance companies, allowing our doctors to provide the best treatment possible instead of that dictated by an insurance company.
We require payment at the time of service unless a complex treatment plan exits. For complex cases, we are pleased to make special financial arrangements.
For patients with indemnity or PPO plans who can see the dentist of their choice rather than choosing one from a list, we will happily submit claims to the insurance company on their behalf. The patient is responsible for the difference between the amount the insurance company pays and our fees. Most insurance companies pay us directly and we will estimate your portion of the fees upon check-out. For your convenience, we accept Visa, Mastercard and Discover.
The following insurance providers do not pay us directly and instead reimburse the patient: MAMSi, BCBS Federal, BCBS of Maryland, CIGNA subscribers employed with Veritas, and Dental Business Operations with Carefirst BlueCross BlueShield. Patients with these insurances are required to pay for their treatment in full at the time of service.
